This page offers practical tools and guidance to support you through the mandatory mediation process. The templates can be used to begin, respond to, or report on the results of your mediation.
Notice of Submission to Mediation Template
Use this template to submit a dispute to mediation by inviting one or more parties to participate.
The responding party is expected to respond within 30 days of this template being sent. A copy of this completed template can be used by any party to move to arbitration if no mediator is selected after 60 days.
Response to Notice of Submission to Mediation Template
Use this template to respond to a Notice of Submission to Mediation by either accepting or rejecting the invitation to mediate. If you accept, you can also propose how the mediation might be held. You may use this same template to reply to a response and agree to any alternative mediators.
Mediation Report Template
You and your parties’ selected mediator may use this template to document the outcome of your mediation, including whether it was held. If mediation failed or did not happen, this template can serve as a notice that your mediation failed. This notice is required so you can submit the dispute to arbitration and move your issue along within 30 days of the notice.
